To identify if there are clinical, spirometric and radiographic differences in COPD patients due to biomass and tobacco. MATERIAL AND METHODS: An observational, cross-sectional analytical study was carried out, a survey was used and clinical records of 242 patients diagnosed with COPD at the Hospital Belén de Trujillo were collected. To calculate the sample size, the sample size statistical formula was used to determine the population proportion. RESULTS: In relation to gender, women were found in greater proportion in the biomass group and men in the tobacco group. In the clinical variables: ¬¬ dyspnea, chronic bronchitis, snoring or wheezing was found in a greater proportion in the biomass group than in the tobacco group. In relation to the spirometric variable FEV1/FVC, it was found that the degrees with greater severity occurred in the biomass group than in the tobacco group. Regarding the radiographic variables: Emphysema, a higher incidence was found in exposure to tobacco than by biomass; in bronchiectasis and atelectasis, it was found in a greater proportion due to exposure to biomass than to tobacco
