Purpose: The aim of this study to demonstrate that the high red cell distribution width (RDW) is a prognostic factor for appendicular perforation in patients with acute appendicitis at Hospital Militar Central. Methods: This analytical, observational, bivariate and multivariate case-control retrospective study has a population that consist in 156 patients with acute appendicitis according to established inclusion and exclusion criteria distributed in two groups: 52 patients with appendicular perforation and 104 without it; Odds ratio, chi square of Pearson for categorical variables and U de Mann-Whitney test for quantitative variables were used in this study. Results: The frequency of high red cell distribution width in patients with acute appendicitis and appendicular perforation was 38/52 = 73%. The frequency of high red cell distribution width in patients with acute appendicitis without appendicular perforation was 51/104 = 49%. In the analysis, the elevated RDW was a prognosis factor of appendicular perforation in acute appendicitis (OR=2.82, p <0.05). In the multivariate analysis using logistic regression, high RDW (p=0.004) and plaquetopenia (p=0.005), were statistically prognostic factors related to appendicular perforation in patients with acute appendicitis. Conclusion: The high red cell distribution width is a prognostic factor for appendicular perforation in patients with acute appendicitis at Hospital Militar Central.
