Introduction: Children are a vulnerable population to develop dengue complications more frequently. There are risk factors such as previous infection, sex, viral strain, among others, described mainly in adults. Excess weight defined as a high body mass index could be one of the factors related to complications in children with dengue. Objectives: To determine whether a high body mass index is a risk factor for developing warning signs in children aged 2 to 15 years with dengue. Materials and methods: Observational, analytical study of cases and controls. 117 children were evaluated, 39 cases with dengue and warning signs and 78 controls with dengue and without warning signs. The body mass index among other demographic variables was determined in each group. Xi squared and correlation were used in the analysis. Results: The average age of the cases was 10.3 ± 3.3 years compared to 8.7 ± 3.8 years in the controls (p <0.05). The male sex predominated in the cases (64.1% vs 38.5% p <0.01). 53.8% of the cases presented a high body mass index compared to 38.5% of the controls (OR 1.9, 95% CI 0.9-4.1, P> 0.05). The most frequent warning signs were platelet low (46%), abdominal pain (33.3%) and high hematocrit (23.1%) Conclusions: The high body mass index is not related to the presence of warning signs in children with dengue from 2 to 15 years of age
