Abstract: Determinar cuáles son los factores de riesgo que incrementa la
probabilidad de mortalidad postoperatoria en pacientes de la cuarta edad con colecistitis
aguda del Hospital de Alta Complejidad “Virgen de la Puerta” durante el tiempo
comprendido entre enero del 2015 a septiembre del 2022.
MATERIAL Y METODO: Se diseñó un estudio de Casos y Controles, donde la muestra
a estudiar fueron adultos mayores de la cuarta edad, es decir aquellos que tengan 80
años de edad, atendidos en los últimos 8 años (2015 - 2022) en el Hospital de Alta
Complejidad Virgen de la Puerta, de esta muestra se formó dos grupos, el grupo casos,
los que fallecieron, y el grupo control, los que sobrevivieron. Se recolecto la
información de variables sociodemográficas, clínicas, de laboratorio, quirúrgicas y
complicaciones postoperatorias, que fueron las variables exposición, y la variable
resultante fue la mortalidad. En el análisis estadístico se usó la prueba Chi Cuadrado
(X2), Test de Fisher para variables categóricas y t de student para variables
cuantitativas, con una diferencia estadística menor al 5% (p < 0.05) así como también la
medida del Odds ratio.
RESULTADOS: En el análisis bivariado el tiempo de enfermedad, el grado de
colecistitis, el nivel de ASA y la Diabetes mellitus tuvieron asociación con la
mortalidad postoperatoria, pero en el análisis de regresión logística la Diabetes mellitus
ya no tuvo asociación y los pacientes que presentaron grado III de colecistitis aguda
tuvieron 4.33 veces mayor riesgo de mortalidad a comparación del grupo de pacientes
con grado II (ORa 4.333 (IC 95%: 1.701-11.038) p<0.05). Los pacientes con tiempo de
enfermedad >58 horas tuvieron mayor riesgo de fallecer a comparación de los pacientes
que tuvieron menor tiempo de enfermedad (ORa 1.013 (IC 95%: 1.003–1.023) p<0.05).
Los pacientes con ASA III tuvieron mayor riesgo de fallecer a comparación de los
pacientes con ASA II (ORa 12.166 (IC 95%: 11.477–12.855) p<0.05).
CONCLUSIONES: El tiempo de enfermedad, el grado de colecistitis aguda y el nivel
de ASA de los pacientes de la cuarta edad con colecistitis aguda si fueron factores de
riesgo para mortalidad postoperatoria
To determine which are the risk factors that increase the probability of postoperative mortality in elderly patients with acute cholecystitis at the Hospital de Alta Complejidad ““Virgen de la Puerta““ during the period from January 2015 to September 2022. MATERIAL AND METHOD: An observational, retrospective study of Cases and Controls was designed, where the sample to be studied were older adults of the fourth age, that is those who have 80 years of age, attended in the last 8 years (2015 - 2022) in the Hospital de Alta Complejidad Virgen de la Puerta, from this sample two groups were formed, the case group, those who died, and the control group, those who survived. Information was collected on sociodemographic, clinical, laboratory, surgical and postoperative complications variables, which were the exposure variables, and the resulting variable was mortality. In the statistical analysis we used the Chi-square test (X2), Fisher's test for categorical variables and Student's t-test for quantitative variables, with a statistical difference of less than 5% (p < 0.05) as well as the Odds ratio.
