To compare the presence of lung injury in preterm infants less than 34 weeks gestational age with respiratory distress syndrome using conventional mechanical ventilation versus continuous positive airway pressure. METHODS: The electronic research of articles was carried out in Medline-Pubmed, Scopus, Web of Science and Embase. Articles from indexed journals in Spanish and English were taken into account, without any publication date restriction. We obtained 908 articles, 160 were marked as ineligible by Rayyan's automation tool, being eliminated. Remaining 748 articles, 126 were detected as duplicated and were excluded. Therefore, a total of 622 articles were reviewed by title and abstract and later, 22 as full text. Finally, 7 articles met all the requirements and were included in this review, 3 of them were trials and 4 cohorts. The risk of bias was made using RoB2 and Newcastle-Ottawa scales, respectively. Statistical analysis was carried out in RevMan 5.4 RESULTS: The use of CPAP in premature infants under 34 weeks with respiratory distress syndrome was shown to reduce the risk of pulmonary injury events compared to the use of mechanical ventilation CONCLUSIONS: The present review affirms that the use of CPAP reduces the risk of lung injury events compared to mechanical ventilation in the treatment of preterm infants less than 34 weeks with respiratory distress syndrome.
