Abstract: Desarrollar y validar una escala con un puntaje propuesto que pronostique complicaciones de preeclamsia.
Material y métodos: Este estudio es observacional, de casos y controles. Se extrajeron los datos correspondientes a edad materna, edad gestacional, saturación de oxígeno, número de plaquetas, volumen plaquetario medio, creatinina sérica, TGO o AST, LDH, líquido amniótico y peso fetal estimado que son las variables componentes de esta escala. Se evaluaron un total de 195 historias clínicas de gestantes con el diagnóstico de preeclampsia atendidas en el Servicio de Obstetricia del Hospital de Alta Complejidad Virgen de la Puerta en el periodo entre enero de 2017 y diciembre de 2021. De todas estas 65 desarrollaron complicaciones y las otras 130 no lo hicieron. Con la prueba Chi Cuadrado se determinó la significancia de las variables y con la curva ROC el puntaje de corte con el que se puede pronosticar complicaciones de preeclampsia.
Resultados: En cuanto a la edad se encontró 46.2% añosas en las pacientes con complicaciones y solamente el 29.2% añosas en el grupo control (p<0.05). Asimismo, al evaluar las complicaciones según edad gestacional antes de las 37 semanas, peso fetal menor al percentil 10 y los indicadores microbiológicos de laboratorio también se observó una diferencia estadística altamente significativa, a excepción del volumen plaquetario medio. Y distinguiendo una mayor diferencia porcentual en DHL ≥ 400. Se evaluaron los puntajes de la escala predictora de complicaciones según la curva ROC y se identificó como punto de corte cuando esta escala alcanza un puntaje mayor o igual a 5.
Conclusión: Esta escala predice efectivamente el desarrollo de complicaciones en pacientes con preeclampsia.
