Laparoscopic cholecystectomy has shown different advantages over open surgery. However, there are relative contraindications that could alter the results of the laparoscopic approach. Objective: To compare the effectiveness of laparoscopic versus open cholecystectomy in patients with previous abdominal surgery with respect to operative time, the frequency of intraoperative and postoperative complications and the length of hospital stay. Methods: We perform a systematic review using mainly five databases. The selection criteria were analytical observational studies that compare the effectiveness of laparoscopic versus open cholecystectomy in patients with a history of previous abdominal surgery. The outcomes studied were: operative time, intraoperative and postoperative complications and length of hospital stay. The study selection, date extraction and risk of bias assessment was performed by two authors. We planned to analyze the dichotomous date with the difference of means and standard deviation, and for continuous data with odds ratio or relative risk. Results: A total of 230 articles were collected; however, only 1 study was included in the review. This study evaluated 58 patients with a history of gastrectomy who were operated by laparoscopic or open cholecystectomy, 34 and 24 patients respectively. Compared to open cholecystectomy, the laparoscopic approach showed a significant decrease in hospital stay (P= 0.009) and the incidence of complications (P= 0.038); however, there was no significant difference in operative time (P= 0.624). In the multivariable logistic regression analysis, laparoscopic cholecystectomy was an independent factor related only to the length of hospital stay (adjusted OR 0.057; 95% IC: 0.004 – 0.74). Conclusions: Comparative statistical synthesis with other studies was not possible. The only article included in the review reports that laparoscopic cholecystectomy is a significant independent factor related to the duration of hospital stay ant not to the operative time or the incidence of complications
