Objective: To determine the effectiveness of warfarin versus the aspirin clopidogrel combination in preventing cerebrovascular disease in patients with atrial fibrillation at Piura Regional Hospital. Material and methods: Retrospective cohort analytical study in patients diagnosed with atrial fibrillation at the Cayetano Heredia Hospital between 2016- 2020. Patients were included in the cohort exposed to warfarin and aspirin clopidogrel by means of cluster-stratified randomization. Curves were made in Kaplan-Meier graphs. The Hazard Ratios (HR) analysis was performed in STATA version 15.0 for the raw and adjusted analysis, considering a 95% confidence level. Results: The incidence rate of cerebrovascular events was 107 cases per 1000 person-months at a follow-up time of 31 months. Of 140 patients, those who belonged to the Warfarin-exposed cohort (n = 70) had a higher frequency of type 2 diabetes mellitus (p = 0.001) and previous vascular disease (0.01). In the multivariate analysis, it was found that the use of warfarin was significantly associated with a 79% reduction in risk of having a cerebrovascular event, after adjusting for the study variables. Conclusions: The use of warfarin was associated with a significant reduction in the risk of developing a cerebrovascular event compared to the use of aspirin clopidogrel in patients with atrial fibrillation
