COVID-19, as an emerging global health threat, posed several challenges, one of which was prevention through the development and production of effective vaccines, developing vaccination campaigns, considering that as any procedure for a vaccine against Covid-19, there may be adverse effects. Objective: The objective of this research was to determine whether the history of SARS-COV-2 infection produces a greater generation of adverse effects after the vaccination process in the population of Piura during the year 2021. Material and methods: A analytical, observational, prospective, not experimental, cohort study was carried out in which 125 persons with a history of SARS COV-2 infection were included, with the addition that they had to meet the criteria for vaccination against COVID-19, in order to form the group of exposed. An anonymous survey on a virtual platform was used to collect these data. Results: The study showed that a patient with a history of SARS-COV-2 infection is 22.7 times more likely to have a post-vaccination adverse event days later compared to one without a history of SARS-COV-2 infection (RR: 22.7; IC:95%; p<0.001). In addition to 22.7 times more likely to have more than 2 days duration of adverse effect (RR: 22.7; IC:95%; p<0.001) and 34 times more likely to have treatment (RR: 34; IC:95%; p<0.001). Although we were not able to obtain a measure of association for severity and outcome of the adverse effect (RR: 0; IC:95%; p<0.001), the most frequent type being mild severity and recovery outcome. It was also found that out of 17 adverse effects, 10 (59%) were associated with a history of SARS-COV-2 infection, showing a p<0.001 in the analysis for the adverse effects of headache, insomnia, fatigue, paresthesia, somnolence and palpitations. Conclusions: It is concluded that a history of SARS COV- 2 infection is associated with a greater generation of adverse effects following the vaccination process in the population of Piura during the year 2021.
