Preeclampsia is a major maternal health problem worldwide, being responsible of a considerable rate of severe neonatal morbidity and mortality. Due to this high mortality, it is very important to identify those factors related to its early diagnosis and prognosis. Objective: To determine if the lymphocyte platelet reduced index is a factor of severity in preeclampsia at the Hospital Belén of Trujillo. Material and methods: A retrospective, analytical, sectional, cross-sectional study was realized, where 245 pregnant women with preeclampsia were included, according to selection criteria, being separated into 2 groups: preeclampsia with severe features and preeclampsia without severe features; applying the prevalence ratio, and the chi-square statistical test. Results: The frequency of gestational age less than 37 weeks and maternal obesity were significantly higher in the group of preeclampsia with severe features (p < 0.05), the frequency of preeclampsia with severe features in pregnant women with decreased lymphocyte platelet index was 110/131 = 84%, the frequency of preeclampsia with severe features in pregnant women with a non-decreased lymphocyte platelet index was 34/114 = 30%, the decreased lymphocyte platelet index is a factor of severity in preeclampsia with a prevalence ratio of 12.32, which was significant (p<0.05). Conclusions: the decreased lymphocyte platelet index is a severity factor in preeclampsia at the Hospital Belén of Trujillo
