Objective: To determine whether the Chole-Risk Score predicts postoperative complications in adult patients with acute calculous cholecystitis undergoing early laparoscopic cholecystectomy. Methods: A cross-sectional analytical study was conducted at the Virgen de la Puerta High Complexity Hospital between January 2018 and December 2022. The Chole-Risk Score, which considers variables such as prior abdominal interventions, comorbidities, predictors of concomitant biliary duct disease, and predictors of difficult cholecystectomy, was used. The medical records of 105 patients were reviewed, and postoperative complications were classified according to the Clavien-Dindo classification ≥ II. Results: The prevalence of postoperative complications was 28.57%. No mortality was recorded in the study. Significant differences were observed in several clinical variables related to postoperative complications, with age, the presence of type 2 diabetes mellitus, hypertension, leukocyte count, blood glucose, ASA score, operative time, and hospital stay being noteworthy. The Chole-Risk Score and its relationship with postoperative complications showed a statistically significant association (p = 0.001). As the Chole-Risk Score increased, the proportion of postoperative complications also increased. Conclusion: The Chole-Risk Score proved to be a significant predictor of postoperative complications in adult patients undergoing early laparoscopic cholecystectomy for acute calculous cholecystitis. These findings emphasize the importance of thorough preoperative assessment and careful management of these patients to minimize the risk of complications
