Abstract: Determinar cuáles son los factores asociados con la recurrencia
de Cáncer de mama luego de la cirugía conservadora en pacientes del
hospital Virgen de la Puerta en el periodo 2014-2019.
Método: Es un estudio observacional, analítico, retrospectivo, transversal
y correlacional predictivo. La investigación contempló a 110 pacientes
diagnosticadas con cáncer de mama y tratadas con cirugía conservadora
en el hospital Virgen de la Puerta en el periodo 2014-2019, de las cuales
se obtuvo la información pertinente de los historiales clínicos. El análisis
estadístico fue realizado en el software SPSS versión 25 y se aplicaron los
estadísticos de chi cuadrado y razón de riesgo.
Resultados: El 92,7% de las pacientes evaluadas tuvo más de 35 años, el
80,9% presentó menopausia, el 55,5% sufrió una distancia negativa del
margen quirúrgico, el 90% tuvo un tipo histológico ductal, el 65,5% presentó
un compromiso ganglionar mayor a 5, al 87,3% se le realizó radioterapia
adyuvante, el 75,5% tuvo un estadio clínico III-IV, el 70,9% tuvo un tumor
con un tamaño mayor a los 2cm y al 73,6% se le practicó una tumorectomía.
La tasa de prevalencia respecto a la recurrencia en pacientes de cáncer de
mama tras cirugía conservadora es del 11,8%. A su vez, el compromiso
ganglionar (p=0,005) y el estadio clínico (p=,0009) se relacionan con la
recurrencia del cáncer.
Conclusiones: El mayor compromiso ganglionar y el estadio clínico
avanzado se relacionan como factores protectores en la recurrencia de
cáncer de mama tras la realización de cirugía conservadora
Determine requirements are the factors associated with the recurrence of breast cancer after conservative surgery in patients of “Hospital Alta complejidad Virgen de la puerta” in the period 2014-2019. Material and Methods: It is an observational, analytical, retrospective, cross-sectional and correlational predictive study. The research included 110 patients diagnosed with breast cancer and treated with conservative surgery at the Virgen de la Puerta hospital in the period 2014-2019, from whom the relevant information was obtained from the medical records. The Statistical analysis was performed in SPSS version 25 software and chi square and risk ratio statistics were applied. Results: 92.7% of the patients evaluated were over 35 years old, 80.9% had menopause, 55.5% had a negative distance from the surgical margin, 90% had a ductal histological type, 65.5% presented lymph node involvement greater than 5, 87.3% underwent adjuvant radiotherapy, 75.5% had a clinical stage III-IV, 70.9% had a tumor larger than 2cm, and 73% .6% underwent a lumpectomy.The prevalence rate regarding recurrence in breast cancer patients after conservative surgery is 11.8%. In turn, lymph node involvement (p=.005) and clinical stage (p=.009) are related to cancer recurrence. Conclusions: Greater lymph node involvement and advanced clinical stage are related as protective factors in the recurrence of breast cancer after conservative surgery.
