Objective: To determine complications associated with the type of surgery in Benign Prostatic Hyperplasia at Hospital III-1 José Cayetano Heredia. Methodology: A cross sectional retrospective analytical study was conducted, using secondary data collection. The Poisson formula and log-link function with Chi2 test were utilized. Additionally, complications were considered as a cluster group, adjusted for age, hospitalization days, and participants' medical history. Results: In the present study 392 patients underwent open adenomectomy, where it was found that the most frequently used surgical technique was retropubic prostatic adenomectomy, used in 54.08% of the cases, while the transvesical technique was used in 45.9%. The age of the patients operated by the retropubic technique ranged from 52 to 84 years, with a mean of 71.16 years and a SD of 5.4 years, while the age of the patients operated by the transvesical technique ranged from 58 to 82 years with a mean of 70.61 and a SD of 5.6. Among the mediated complications, the most frequent coincidentally found in both techniques was UTI, which occurred in 11.1% of patients operated on by the transvesical route and in patients operated on by the retropubic route in 16.03%, followed by incontinence and urinary incontinence in patients operated on by the retropubic route. 03%, followed by urinary incontinence which occurred in patients operated by transvesical route with 2.2% and in patients operated by retropubic route with 4.71%, also operative wound infection in A.P.T.V had 2.2% and in A.P.R.P 1.88% respectively for both techniques.Conclusions: A significant relationship was found between hospitalization duration and complications of open surgery for Benign Prostatic Hyperplasia treatment at Hospital III-1 José Cayetano Heredia in 2019 (p<0.05). Retropubic prostatectomy was performed in 54.08% of cases (106 patients), while the transvesical technique was used in 45.9% (90 patients). The average hospital stay was 9.91 days for the transvesical technique and 8.27 days for the retropubic technique. There were 68 complications (64.13%) with the retropubic technique and 58 (64.2%) with the transvesical technique, with bleeding being the most frequent complication in both, at 37% and 35% respectively
