To determine if gamma glutamyl transpeptidase is more useful than total serum bilirubin in the diagnosis of choledocholithiasis. Material and methods: A retrospective study of diagnostic tests was conducted in which 182 patients diagnosed with cholelithiasis are included, according to the selection criteria they were divided into one group with choledocholithiasis and the other group without choledocholithiasis, subsequently the area under the curve was calculated. Results: The frequency of high alkaline phosphatase and hipertransaminasemia in the group with choledocholithiasis is statistically significant comparison of the group without choledocholithiasis, being p <0.05. The values obtained from the GGT in the diagnosis of choledocholithiasis were the following: sensitivity (83%), specificity (79%), positive predictive value (43%), negative predictive value (96%) and the diagnostic accuracy was 84%. The values obtained from total serum bilirubin in the diagnosis of choledocholithiasis were the following: sensitivity (73%), specificity (69%), positive predictive value (30%), negative predictive value (93%) and diagnostic accuracy was 74%. Gamma glutamyl transpeptidase is more useful than total serum bilirubin in the diagnosis of choledocholithiasis. Conclusion: Gamma glutamyl transpeptidase is more useful than total serum bilirubin in the diagnosis of choledocholithiasis
