To determine if the level of adherence to a protocol for premature rupture of membranes influences therapeutic success. Material and methods: A retrospective cohort study was performed. in 373 pregnant women with PROM, hospitalized in the HBT between January 2017 and December 31, 2019; in whom a documentary investigation was carried out with a checklist, to verify the level of adherence to the RPM protocol of the HBT. 65% was considered as the cut-off point to differentiate low and high level of adherence. Results: 88% of the cases (329) presented a high level of adherence, of which 57% (213) achieved therapeutic success. On the other hand, 12% (44) had a low level of adherence and of these, therapeutic success was only achieved in 3% (12). with a statistical significance < 0.01 with a RR of 0.485 IC 95% (0.35 to 0.66), which makes it a protective factor that favors therapeutic success. 22.7% of patients with low level of adherence were complicated by chorioamnionitis and 63.6% of neonates presented neonatal sepsis. Conclusion: The high level of adherence to the PROM protocol significantly influenced therapeutic success, in addition, it decreased the prevalence of Chorioamnionitis and neonatal sepsis in pregnant women with premature rupture of membranes hospitalized in the HBT between January 2017 and December 2019
