Abstract: El síndrome de Guillain-Barré (SGB) es una polirradiculoneuropatía inflamatoria aguda
desmielinizante de evolución rápida y potencialmente fatal, de etiología autoinmune y
comúnmente precedida de un cuadro infeccioso gastrointestinal o respiratorio previo.
Suele caracterizarse por síntomas que empiezan con debilidad muscular flácida y
bilateral, simétrica y ascendente, acompañada de pérdida de reflejos; sin embargo, puede
presentarse con otras manifestaciones que pueden dificultar el diagnóstico y orientarlo
hacia otra patología.
Se presenta el caso de un paciente varón de 68 años de edad, sin antecedentes de
enfermedades o infecciones previas, que cursó con dolor torácico de un día de evolución,
a lo que posteriormente se le agregó debilidad muscular que inició por miembros
superiores y progresó a miembros inferiores, asociados a numerosas complicaciones. El
estudio de líquido cefalorraquídeo (LCR) no mostró alteraciones; sin embargo, se
confirmó el diagnóstico de SGB atípico con la electromiografía.
Guillain-Barré syndrome (GBS) is an inflammatory demyelinating acute polyradiculoneuropathy of rapid evolution and potentially fatal with autoinmune etiology and history of previous gastrointestinal or respiratory infection , which usually starts with characteristic symptoms like flaccid, bilateral, symmetrical and ascending muscular weakness accompanied by loss of reflexes; however, it can present with other manifestations that can make diagnosis difficult and orient it towards another pathology We present the case of a 68-year-old male patient, with no history of previous illnesses or infections, who presented with chest pain of one day of evolution, which was later added muscle weakness that started by upper limbs and progressed to lower limbs, associated with numerous complications. The cerebrospinal fluid study showed no alterations; however, the diagnosis of atypical GBS with electromyography was confirmed.
