Objective: To determine whether there is an association between multimorbidity and mortality in patients with chronic kidney disease (CKD) on hemodialysis. Materials and methods:An observational case-control study was conducted in 680 patients with CKD who started hemodialysis at the Víctor Lazarte Echegaray Hospital from 2019 to 2023. The cases corresponded to 340 deceased patients and the controls to 340 patients who survived a minimum time of 6 months. The risk factor evaluated was the number of associated comorbidities. Results: Two or more comorbidities (multimorbidity) were present in 268 (78.8%) deceased patients and in 196 (57.6%) surviving patients (p= <0.001) and were associated with mortality with OR: 2.73; 95% CI: 1.95-2.83. Three or more comorbidities were present in 150 (44.1%) deceased patients and in 59 (17.4%) surviving patients (p= <0.001) and were associated with mortality with OR: 3.76; 95% CI: 2.64-5.35. Four or more comorbidities were present in 46 (13.4%) deceased patients and in 6 (1.8%) surviving patients (p= <0.001) and were associated with mortality with OR: 8.71; 95% CI: 3.67-20.68. There were > 3 points in the Charlson comorbidity index in 225 (66.2%) deceased patients and 109 (32.1%) surviving patients (p=<0.001) and it was associated with mortality with OR: 4.15; 95% CI: 3-5.71. Conclusions: Multimorbidity was associated with mortality in patients with chronic kidney disease on hemodialysis
