Introduction: Acute coronary syndrome (ACS) is one of the most frequent group of heart diseases in the world, its high mortality requires increasingly better elements to predict its prognosis. Objective: To demonstrate that the red blood cell distribution width (RDW) is a predictor of mortality in patients with ACS. Method: Study of cases and controls where 138 clinical histories (46 cases and 92 controls) of patients diagnosed with ACS, hospitalized in the Internal Medicine department, of Hospital Militar Central between 2014 and 2018. The Odds Ratio (OR) for the bivariate analysis was calculated and the multivariate analysis was performed by logistic regression for the analysis of the covariates. Results: The RDW> 14.5% occurred in 65% of deaths and 24% of controls, being the only independent risk factor for mortality due to ACS after adjusting the confounding variables (ORa = 6.03; 95% CI = 2.69-13.52). Conclusion: red blood cell distribution width is a predictor of mortality in patients with acute coronary syndrome.
