The objective of the study is to verify if hyperbilirubinemia is a valid factor to predict appendicular perforation. Material and methods: The study is prospective, of a diagnostic test type, the sample included data from 112 medical records, which fulfilled the selection criteria, which were recorded in data record sheets, then a database was created in Excel 2013 , to be subsequently processed by the statistical package SPSS25; And so give an analysis for sensitivity, specificity, positive predictive value, negative predictive value, area under the curve for total serum bilirubin, and thus demonstrate the prognostic validity of hyperbilirubinemia in appendicular perforation. Results: An association was observed between hyperbilirubinemia and appendicular perforation in patients suffering from acute appendicitis, as well as 15.2% of patients presenting with hyperbilirubinemia and appendicular perforation, 57.7% without any of the 2 conditions; The best cut-off point for serum bilirubin values to predict appendicular perforation of 0.94 mg / dl, Fuel sensitivity and specificity of 45.9% and 78.7% respectively; The positive predictive value and the negative predictive value were 51.5% and 74.7% respectively. The variables with statistical association with appendicular perforation are neutrophils and age in the third decade of life significantly; and anemia in a highly significant way, on the contrary, leukocytes, CRP and gender no significant association relationship. Conclusion: The validity of hyperbilirubinemia as a predictor of appendicular perforation was 70.3%, corresponding to an intermediate prognostic accuracy. The diagnosis of perforation can be suspected when bilirubin levels are> 0.94 mg / dl. Therefore we recommend the use of hyperbilirubinemia for the diagnosis of appendicular perforation.
