Introduction: Community-acquired pneumonia (CAP) is the infectious respiratory disease with the highest mortality rate worldwide, being able to identify prognostic markers would help in the preventive management of death. Objective: to demonstrate that the C-reactive protein / albumin index is a prognostic marker of mortality in community-acquired pneumonia. Method: Study of diagnostic tests in which 126 clinical records of patients with CAP, hospitalized in the Hospital Belen de Trujillo, 2014-2018, were reviewed, evaluating the PCR / albumin index (IPA) upon admission and the death of the patient within 30 days of hospitalization. The area under the ROC curve of the best cut-off point was determined and the adjusted OR was calculated by multivariate analysis establishing the risk of death independently. Results: The optimal IPA cutoff point was 2.5, with an area under the curve of 0.891 (95% CI: 0.838-0.944), sensitivity of 87%, specificity of 81%, PPV of 67% and NPV of 93%, as well as It was an independent marker of death by NAC, increasing this risk by 10.67, higher than CRP alone (ORa: 7.15). Conclusion: the C-reactive protein / albumin index is a marker of mortality in NAC.
