Objective: To determine if the inversion of the neutrophil/lymphocyte ratio is a factor associated with dengue in adult patients with febrile syndrome, in a reference hospital in northern Peru. Material and methods: The present study had an analytical design, of cases and controls, retrospective, carried out in 150 patients older than 15 years of age, who were admitted to the Hospital JAMO II-2 Tumbes, with a diagnosis of febrile syndrome, treated during January 2018 and July 2020. at the JAMO II Regional Hospital – Tumbes. For associations of quantitative data, the Student's T test was used if the quantitative data showed a normal distribution, or in any case the U–Mann Whitney statistical test if the analyzed distribution did not show normality. The Odds Ratio allowed to evaluate the strength of association between the factor under study: Inversion of the neutrophil/lymphocyte ratio and the event: dengue in adult patients with febrile syndrome. Results: Of the patients without dengue, no patients (0.0%) and of those with dengue, 46 (92.0%) had an inversion of the neutrophil/lymphocyte ratio, this being a significant risk factor for dengue, according to the multivariate analysis (ORa = 101.76; 95% CI = 14.65 – 691.48; p = 0.01). Conclusions: The inversion of the neutrophil/lymphocyte ratio is a factor associated with dengue in adult patients with febrile syndrome, in a reference hospital in northern Peru
