Background: Lymphangitic carcinomatosis is a rare metastatic disease associated with different types of malignant neoplasms, including lung cancer with an incidence of less than 6%. It has a nonspecific clinical presentation and a high mortality rate. In this clinical case, a young man presented with a nonproductive cough, progressive dyspnea, and chest pain; a previously diagnosed cardiac tamponade. The study excludes infectious, metabolic and autoimmune causes. Due to the tomographic findings compatible with lymphangitic carcinomatosis, a neoplastic etiology is suspected. The results of the pericardial and lung biopsy concluded a primary malignant lung tumor. Due to advanced disease, the patient receives palliative treatment and dies 68 days after admission. Conclusion: Lymphangitic carcinomatosis is a late manifestation of a neoplasm, its clinical presentation is varied so its diagnosis is difficult with the need for pathological anatomy. Once the diagnosis is established, the prognosis is bleak and the mortality rate is high.
