To determine the effectiveness of Early compared to Late Cranioplasty in reducing post-surgery complications in post-operative patients with decompressive craniectomy in the Neurosurgery service of the Teaching Regional Hospital of Trujillo during the 2014-2019 period. Material and Methods: An analytical, observational, cohort, retrospective study was carried out. The study population consisted of 70 patients exposed to decompressive craniectomy according to established inclusion and exclusion criteria distributed in two groups: with Early Cranioplasty or Late Cranioplasty; relative risk and chi square test were calculated. Results: No significant differences were found regarding the variables age, operative time, gender or in-hospital mortality between patients undergoing Early Cranioplasty compared to Late Cranioplasty (p > 0.05). No significant differences were observed regarding the frequency of surgical site infection between patients undergoing Early Cranioplasty compared to Late Cranioplasty (p > 0.05). No significant differences were found regarding the frequency of hydrocephalus between patients undergoing Early Cranioplasty compared to Late Cranioplasty (p > 0.05). Fewer post-surgical complications such as seizures and cerebral hemorrhage were observed in the Early Cranioplasty group compared to the Late Cranioplasty group (p<0.05). Conclusions: Fewer post-surgical complications were observed, such as: seizures and cerebral hemorrhage in the Early Cranioplasty group compared to the Late Cranioplasty group.
