OBJECTIVE: The purpose of this study was to determine the relationship between malocclusion, expectation of pain perception and attitude towards orthodontic treatment in teenagers aged 12 to 17 years. MATERIAL AND METHOD: This study was cross-sectional, descriptive, prolective and observational. The sample population was made up of 128 students who met the inclusion criteria. The Dental Aesthetics Index (DAI) was used as an instrument to evaluate malocclusions, and two questionnaires were used to evaluate the expectation of pain perception and attitude towards orthodontic treatment. The data obtained were entered into an Excel database to later be imported into the IBM SPSS Statistics 26 program. The degree of correlation between the 3 study variables was determined using Spearman's correlation coefficient. A significance level of 5% was considered. RESULTS: In the study it was found that there is only a relationship between expectation of pain perception and attitude towards orthodontic treatment (rs=0.510, p= 0.000 < 0.001). The prevalence of malocclusions was that 58.6% had normal malocclusion, 28.01% had definite malocclusion, 12.5% had severe malocclusion, and 0.8% had very serious malocclusion. It was found that there is a relationship between expectation of pain perception and attitude towards orthodontic treatment in schoolchildren according to sex and age. CONCLUSION: There is only a relationship between expectation of pain perception and attitude towards orthodontic treatment in schoolchildren aged 12 to 17 years
