Introduction: COVID-19 is a disease that can cause serious complications, including stroke. Some research suggests that COVID-19 may increase the risk of stroke due to inflammation and clotting associated with the disease. Objective: This study aimed to evaluate the association between COVID-19 and mortality in stroke patients at the José Cayetano Heredia-Piura Hospital in Peru from 2020 to 2021. Methods: A retrospective cohort study of 75 stroke patients was conducted. 31 patients had COVID-19 and 44 did not. The variables studied included age, sex, type of stroke, stroke onset, Imaging findings, length of hospitalization, Glasgow score, and blood pressure. Results: The overall mortality rate was 29.3%. The mortality rate for patients with COVID-19 was 32.3% and for patients without COVID-19 was 27.3%. This difference was not statistically significant (p > 0.05). The type of stroke was a significant risk factor for mortality (p = 0.019). Patients with ischemic stroke were more likely to die than patients with hemorrhagic stroke. Stroke onset was also a significant risk factor for mortality (p = 0.004). Patients with insidious onset stroke were more likely to die than patients with sudden onset stroke. The other variables studied were not significant risk factors for mortality. Conclusions: The results of this study did not show a significant association between COVID-19 and mortality in stroke patients. However, the type of stroke and stroke onset were significant risk factors for mortality
