To determine if there is an association between critical factors and postoperative morbidity and mortality in elective laparoscopic cholecystectomy in the elderly. Materials and method: An observational, analytical, retrospective case-control study was carried out. 235 patients undergoing elective laparoscopic cholecystectomy at the “High Complexity Hospital of La Libertad: Virgen de La Puerta” were included. of 47 patients with complications or deaths and 188 without complications or deaths. Results: Postoperative hospitalization of 2 days and 3 or more days showed a statistical association with complications (p < 0.001; OR=5.43 for 2 days and OR=73.63 for 3 or more days). Surgery lasting longer than 60 minutes was also associated (p=0.036; OR=2.84). There was no association in age, sex, ASA classification, comorbidities, and harmful behaviors. The multivariate analysis confirmed the relationship between hospitalization of 2 days (OR=6.49, p=0.001) and 3 or more days (OR=95.16, p=0.000) with complications, while surgery lasting more than 60 minutes did not show a significant association. (p=0.26). Conclusions: Hospitalization time greater than and equal to 2 days is a risk factor for postoperative complications in older adult patients undergoing elective laparoscopic cholecystectomy
