To evaluate the efficacy of vitamin A in the prevention of bronchopulmonary dysplasia in preterm infants through the synthesis of evidence in a systematic review and meta-analysis. Methods: The search was performed in four medical databases; all those papers that met the following criteria were included: 1) randomized controlled trials (RCT); 2) preterm neonatal patients less than 37 weeks; 3) studies comparing Vitamin A vs. placebo or standard care. Outcomes were incidence of BPD, incidence of sepsis and incidence of retinopathy of prematurity (ROP). Risk of bias analysis was assessed using RoB 2.0. The GRADE technique was used to assess the quality and certainty of evidence for all outcomes. The meta-analysis used the random effects model and inverse variance method. Results: 348 patients were identified among all studies, of whom 180 were male and the mean age was 27.6 weeks gestational age (SD 1.7). In preterm patients, vitamin A does not decrease the incidence of PDB compared to placebo (RR 0.76; 95%CI 0.01-50.84; p=0.56). Similarly, in preterm patients, vitamin A does not decrease the incidence of sepsis compared to placebo (RR 0.69; 95%CI 0.001- 336.9; p=0.59). Finally, in preterm patients, vitamin A does not decrease the incidence of ROP compared to placebo (RR 0.61; 95%CI 0.001-104.1; p=0.43). Conclusions: Overall, in preterm patients, Vitamin A use does not reduce the incidence of bronchopulmonary dysplasia, sepsis, and retinopathy of prematurity. The evidence indicates that there is not a sufficient degree of recommendation to support the usefulness of supplementation to prevent clinical outcomes in these patients.
