Abstract: La COVID-19, causante de una emergencia sanitaria a nivel mundial,
cursa con síntomas predominantemente respiratorios; sin embargo, estudios
recientes han demostrado que los pacientes pueden presentar otros síntomas
extrapulmonares como dolor abdominal, pérdida de apetito, náuseas, vómitos y
diarrea, incluso sin presentar síntomas del tracto respiratorio. El mecanismo por el
cual los pacientes presentan síntomas abdominales podría explicarse debido a la
presencia de los receptores ECA-2 en el intestino, haciéndolo susceptible a la
infección por el SARS-CoV-2 y generando una hiperinflamación y tormenta de
citocinas, debido a que el intestino es el órgano inmunitario más grande. Por ello,
es indispensable crear una estrategia para identificar de manera oportuna la
infección por la COVID-19, realizando una TAC torácica al mismo tiempo que la
TAC abdominal para un diagnóstico temprano.
Objetivo: Revisar la literatura en relación al abdomen agudo como un síntoma
precoz de la COVID-19.
Métodos: Se realizó una búsqueda bibliográfica en el periodo comprendido entre
enero y octubre del 2020 utilizando como términos claves COVID-19, SARS-CoV 2, dolor abdominal, abdomen agudo. Las bases de datos utilizadas fueron Pubmed
y LILACS, se seleccionaron artículos en inglés y español.
Conclusiones: El dolor abdominal inexplicable debe considerarse como un criterio
en el procedimiento previo al triaje. Esto reducirá potencialmente el riesgo de
transmisión a otros pacientes y al personal del hospital, ya que se han presentado
más casos de pacientes con dolor abdominal desde el inicio o en los primeros días
como parte del cuadro clínico de la COVID-19, siendo en su mayoría de casos
cuadros abdominales que se autolimitan sin necesidad de intervenciones
específicas.
COVID-19, the cause of a global health emergency, causes predominantly respiratory symptoms; however, recent studies have shown that patients may present with other extrapulmonary symptoms such as abdominal pain, loss of appetite, nausea, vomiting, and diarrhea, even without presenting respiratory tract symptoms. The mechanism by which patients present abdominal symptoms could be explained due to the presence of ACE-2 receptors in the intestine, making it susceptible to infection by SARS-CoV-2 and generating hyperinflammation and cytokine storm, due to the fact that the intestine is the largest immune organ. For this reason, it is essential to create a strategy to identify COVID-19 infection in a timely manner, performing a chest CT at the same time as an abdominal CT for an early diagnosis. Objective: To review the literature in relation to the acute abdomen as an early symptom of COVID-19. Methods: A bibliographic search was carried out in the period between January and October 2020 using as key terms COVID-19, SARS-CoV-2, abdominal pain, acute abdomen. The databases used were Pubmed and LILACS, articles in English and Spanish were selected. Conclusions: Unexplained abdominal pain should be considered as a criterion in the pre-triage procedure. This will potentially reduce the risk of transmission to other patients and hospital staff, since more cases of patients with abdominal pain have been presented since the beginning or in the first days as part of the clinical picture of COVID-19, being in its majority of cases abdominal symptoms that are self-limited without the need for specific interventions.
