Determine if there is a relationship between iron reserve levels and the development of gestational diabetes. Material and Methods: A systematic review was carried out in 5 databases of observational studies: cohorts, cases and controls. Two authors performed the article search, data extraction, and bias assessment. The outcome was the development of DMG. Subsequently, the statistical analysis of the selected studies was carried out, for which the fixed effects model was used with the inverse variance method (IV); the relative risk (RR) was made for dichotomous variables and mean difference for continuous variables. Results: Five articles were included in the review and 3 in the meta-analysis, which demonstrated that high ferritin levels in the first trimester of pregnancy increased the risk of developing GDM by 84%. (RR: 1.84, 95% CI 1.32–2.57, P: 0.0003). However, no significant difference was found between the risk of developing GDM in patients with high levels of sTfR, versus low levels during the first trimester (RR: 1.18, 95% CI 0.95 -1.46, P: 0.13). A significant difference was found in the mean difference of ferritin levels in patients with GDM versus without GDM (MD 6.31; 95% CI 2.41 - 10.22; p=0.002); as well as in the mean difference of BMI values before pregnancy in patients with GDM versus without GDM (MD 2.86; 95% CI 2.36 - 3.36; p<0.00001) during the first trimester. Conclusions: High ferritin concentrations in the first trimester of gestation were associated with the risk of GDM (RR: 1.84, 95% CI 1.32–2.57, P: 0.0003).
