Frailty has been shown to be associated with adverse patient outcomes; this concept has been adopted in a frailty index. Objective: To determine if the frailty index is a predictor of morbidity and mortality after laparoscopic cholecystectomy in patients with acute cholecystitis. Material and methods: A retrospective cohort study was conducted. A total of 219 medical records of postoperative laparoscopic cholecystectomy patients were reviewed at the Víctor Lazarte Echegaray Hospital in Trujillo during the period 2017-2021, of which the exposed sample (with a high frailty index) was 55 patients and the non-exposed (with non-high frailty index) was 164 patients, both groups met the inclusion criteria, in which postoperative morbidity, mortality and the incidence of postoperative complications were compared, such as: abscess, fever, post operative bleeding among others. Results: Of the 219 medical records reviewed, 0% postoperative mortality was obtained. Regarding postoperative morbidity, 31 patients had postoperative complications (35.48% high FI vs 64.52% non-high FI; p < 0.001) and 188 patients had no complications (25.53% elevated IF vs 74, 47% non-elevated IF, p < 0.001) with a RR = 4.13; CI 95% (2.17 - 7.87). Conclusions: Frailty is associated with postoperative morbidity in patients who underwent laparoscopic cholecystectomy for acute cholecystitis, but it is not associated with mortality
