Introduction: Acute uncomplicated appendicitis is a frequent cause of consultation in the department of general surgery. Despite this, the use of antibiotics after surgery is not completely defined in this population. Objective: To determine whether the use of antibiotic therapy after appendectomy for uncomplicated acute appendicitis is related to the appearance of operative site infection at 30 days in patients older than or equal to 18 years of age. Materials and Methods: Prospective patients were recruited in the Emergency General Surgery service of the Hospital Nacional Dos de Mayo in the 2018-2019 period. All patients signed an informed consent before moving on to data collection. The population was divided into two exposure groups, in which they received antibiotics or not after surgery. Results: A total of 521 patients were analyzed, 35 (6.7%) patients developed infection of the operative site. These patients had a higher age (41.7 ± 13.5 vs. 32.8 ± 12.5) and a longer hospitalization time (9.1 ± 4.1 vs 2.5 ± 2.3 days) than patients who did not develop infection of the operative site. In the analysis of relative risk, it was identified that the use of antibiotic after surgery does not prevent the appearance of infection of the operative site in this population (RR: 0.89, 95% CI: 0.47 - 1.69, p = 0.722). Conclusions: There is no benefit from the use of postoperative antibiotic therapy to prevent the development of infection of the operative site in patients presenting with uncomplicated acute appendicitis.
