To determine if totally extraperitoneal endoscopic hernioplasty (TEP) is more effective than Lichtenstein hernioplasty in patients with inguinal hernia is not complicated. Materials and methods: A systematic review was conducted, including twenty-seven randomized clinical trials comparing two techniques: Totally Extraperitoneal Endoscopic Hernioplasty (TEP) versus Lichtenstein hernioplasty. Data were extracted and analyzed to compare the efficacy of both surgical techniques for inguinal hernioplasty. Results: A total of 27 RCTs that involving 7313 patients included in the review. The PET technique showed less chronic pain (Relative Risk [RR] = 0.38; 95% CI [0.28 – 0.51]), along with less operative wound infection which is reduced by 52% (RR 0.48; 95% CI). [0.31 – 0.75]), a 37% reduction in hematoma (RR 0.63; 95% CI [0.41 – 0.97]) and less hospital stay in days (MD -0.83; 95% CI [-1.24 - -0.41]) in addition to shorter return time to daily activities (MD -4.74; 95% CI [-6.78 - -2.70]) compared to the Lichtenstein technique. No significant differences were found for recurrence, seroma, scrotal edema, operating time, and length of hospital stay in hours between the two techniques. Conclusions: The TEP technique is more effective in terms of chronic pain, risk of surgical wound infection, and reduction of hematoma formation compared to the Lichtenstein technique.
