To determine the difference in clinical outcomes between urinary infection by ESBL and non-ESBL germs in a Trujillo Hospital. Material and methods: A retrospective cohort study was carried out in 93 patients with urinary tract infection, which were divided according to the presence of ESBL germs or not. The clinical outcome was compared through the chi-square calculation and the relative risk, considering a significant value when p was <0.05. Results: In the group of patients with ESBL infection, it was found that the female gender was significantly higher (p<0.05). In-hospital mortality in patients with ESBL urinary tract infection was associated with anemia (p=0.034). Prolonged hospital stay in patients with urinary tract infection was associated with infection by ESBL germs (75.0% versus 40.3%; p = 0.001; RR 1.31; 95% CI 0.81-2.11) and with the greater mean age of patients (68.6 ± 15.7 versus 61.8 ± 17.5 years; p = 0.048). When performing the multivariate analysis of these, it was found that only the presence of UTI due to ESBL germs was more significant (p = 0.034; RR 1.832; 95% CI 1.046-3.210). No significant association was found on admission to the intensive care unit (ICU). Conclusion: Urinary tract infection by ESBL germs was associated with the female gender, in addition, in-hospital mortality in patients with ESBL infection was associated with anemia, prolonged hospital stay is more frequent in patients with ESBL infection and in older age. >68 years
