In Peru, the INEI conducts a national survey of the satisfaction of users of universal health insurance. This survey aims to assess the perception of user satisfaction with the care received in health centers. One of the sections describes the person's knowledge about their medical diagnosis, 54.90% did not obtain a diagnosis since they only collected results, 34.40% did know, 8.00% were not diagnosed, and 2.70% he does not know The number of people who did not receive or do not know is due to the lack of information about their health history or the lack of a registry that ensures the integrity of it. Currently, the health sector in Peru manages the information of treatments and medical appointments of patients through documents called clinical histories, which are stored physically in file rooms, warehouses, dedicated repositories, etc. or virtually in an information system. Faced with this, by not having organized information or having any way to integrate the clinical history between health entities, the disadvantages of this are: Do not consider relevant information of the patient during the medical consultation, guided by erroneous or incomplete data at the time of diagnosis, etc. Due to the minimal knowledge and little confidence in the use of cloud technologies to store sensitive information about people, entities avoid using these techniques to improve the process of gathering information during medical consultation. For this reason, the purpose of the project is the development and implementation of a technological model supported by a mobile solution and web platform that allows self-management of health history information by the patient and consultation in real time by the doctor respectively. the use of technology in health entities. Likewise, it implies that the health sector, in the long term, does not present difficulties in adapting to changes in the business due to digital induction.
