Objective: To determine if there is an association between altitude above 2260 masl (meters above sea level) and presence of congenital heart disease in pediatric patients under 18 years old in the INSN, Lima-Peru, in the years 2017-2018. Material and methods: Cross-sectional study in children of the outpatient clinic, interconsultation, eco-consultation and hospitalization of the pediatric cardiology service, through surveys of the patients´s mother and review of medical records. The district, city and department where the mothers lived during the pregnancy were registered, to then find the value of the altitude. The presence of congenital heart diseases was certified with the echocardiographic report in the medical records. The associations were evaluated in Poisson regression models with robust variance, and the effects were described as the prevalence ratio (PR). Results: We analyzed 570 cases (median: 2 years (ICR 0 to 6), 51.8% males). The median altitude was 161 masl (157 to 182); the prevalence of congenital heart disease was 68.07% (95% CI 64.1 to 71.7). We found 80.5% of acynotic congenital heart diseases and 19.5% cyanotic. The most common congenital heart diseases were ventricular septal defect (29.0%), persistent ductus arteriosus (17.5%), atrial septal defect (17.0%) and tetralogy of Fallot (3.5%). In the multivariate analysis we found that a pregnancy in regions of more than 2260 masl was associated with a 20% greater probability of developing congenital heart diseases compared to regions located below than 2260 masl (RP: 1.2, 95% CI 1.1-1.3, p = 0.002). The altitude distribution of this study showed that the majority of patients with congenital heart disease were gestated at a altitude of less than 1500 masl. Additionally, those patients with Down syndrome had a significant increase (30%) in the probability of presenting congenital heart disease (RP: 1.3, 95% CI 1.2-1.4, p <0.001). There is no association between the number of congenital heart diseases and the altitude. Conclusions: In a Peruvian pediatric referral center we found that altitude> 2260 meters above sea level was significantly associated with a 20% higher rate of congenital heart disease.
