The objective of the research was to determine the water quality index and heavy metals in the San Juan River, located in the Province of Pasco, Department of Pasco during the period 2012-2018. The development of the research was justified because it is necessary to show the impacts generated by anthropogenic activities in this body of water, in this case being the presence of mining and population activity. For the estimation of water quality, the Water Quality Index (ICA CCME) and the Heavy Metal Index (HPI) were applied, the study was based on the results of the water quality monitoring of the monitoring network of the National Water Authority from 2012 to 2018, at each monitoring point. For the ICA CCME the parameters of pH, OD, Cyanide, Al, Cd, Cu, Fe, Mn, Pb, Zn, C. thermotolerant, E. coli, As, and Hg were used; for HPI, the parameters of Al, Cd, Cu, Fe, Mn, Pb and Zn were used. The study used both quality indices in a total period from 2012 to 2018 and in 2 periods which included the years 2012 to 2015 and from 2016 to 2018. The results found show negative impacts on the San Juan River, the study concluded that the upper, middle and lower part of the San Juan River present a water quality of good, poor and regular respectively, the results showed that for both indices the qualification in 2 periods was the most realistic, compared to the evaluation of the total period that did not evidenced the improvements in water quality, which occurred in the section of the final middle part and lower part of the San Juan river. The uses of water that can be given in the San Juan river according to the qualification obtained with the ICA CCME water quality index, indicate that in the upper part it can be used for aquatic life, water supply, Industrial processes and unrestricted watering; In the middle part, it is only suitable as a receiving body for wastewater discharges due to the degree of contamination that its waters presented; in the middle final and low part its rating was regular and it is recommended to use it for restricted irrigation, manual exploitation of construction materials and recreation with restricted contact
