Objectives: Determine the factors associated with adherence to hand hygiene in health personnel of the hospitals of the III level of care in the Cusco Region, 2021. Methods: An observational, correlational, analytical, cross-sectional, prospective study was carried out. A total of 183 health workers from two level III hospitals in the Cusco Region were studied. Three instruments were used for data collection, the first was the validated instrument with a Cronbach's alpha of 0.804 which evaluates the personal and institutional factors that influence good adherence to hand hygiene, the second instrument used was the elaborated by the World Health Organization in 2009 which measures adherence to hand hygiene and the third instrument used was the Hand Hygiene Technique Monitoring Form prepared by the Management and Quality Directorate of the Regional Management of Cusco which evaluates compliance with the 11 steps and the time involved in the hand hygiene technique. The calculation of absolute and relative frequency was used for quantitative variables, as well as measures of central tendency and dispersion for qualitative variables; in addition; bivariate analysis was used to determine the association between variables using the Chi square; Logistic regression was used to obtain the ORa which were found in the statistical program SPSS v.25.0 for macOS. Results: 183 health workers were evaluated, obtaining adherence in 114 health workers (62.3%). The most important associated factors found are the work department where it was seen that the Pediatrics - Neonatology department obtained the highest adherence with 44 health workers (24.0%, ORa 1.605 <0.000, CI 95%), the occupational group with the highest adherence was the nursing one with 38 nurses (20.8%, ORa 1.285, p=0.006, CI 95%). Other associated factors were age between 41 and 45 years with 24 health workers (13.1%; ORa, p=0.006, CI 95%), years of service between 5 and 10 years with 49 health workers (26.8%, ORa 1.755 , p=0.000, CI 95%), knowledge at 5 moments of hand hygiene for care with 95 health workers (51.9%, ORa 3.625, CI 95%; 1.825 - 7.201), knowledge of the technique of hand hygiene with 92 health workers (50.3%, ORa 2.091, CI 95%; 1.056 - 4.141), allergies with 82 health workers (44.8%, ORa 3.141, CI 95%; 1.680 - 5.874), physical environment with 104 health workers (56.8%, ORa 3.671, CI 95%; 1.581 – 8.524), training with 91 health workers (49.7%, ORa 4.316, CI 95%; 2.236 – 8.330), material resources 123 health workers (48.6% , ORa 3.665, CI 95%, 1.917 – 7.004). Conclusions: There is a correlation between personal and institutional factors (years of service, knowledge of the 5 moments of hand hygiene for care, knowledge of hand hygiene technique, allergies, age, occupational group, adequate physical environment, training, material resources and department where they work) and adequate adherence to hand hygiene in the health personnel of the hospitals of the III level of care in the Cusco Region.
