Introduction: In March 2020 began a health crisis worldwide, infection by a type of coronavirus, COVID-19 generated more than 431 million cases and 5.93 million deaths until February 2022 and in the Cusco region until the same date 193 347 cases were reported with a mortality rate of 11.02 per 10000 inhabitants. In pregnancy, thanks to the physiological and immunological changes associated with COVID-19 infection, there is the possibility of presenting maternal and fetal complications such as HDP (Hypertensive Disorders of Pregnancy). Research studies carried out at sea level show an important association between these two variables; however, there is no evidence that this complication occurs at high altitude. Purpose: To determine whether COVID-19 infection is a risk factor for the development of HDP in pregnant women attended at the Regional Hospital of Cusco, 2021. Materials and methods: The design was case-control study. The sampling was nonprobabilistic and by convenience. For the statistical analysis we used OR and chi-square calculation and for the adjustment we performed a multivariate logistic regression with the variables: prenatal controls, obesity, altitude, immunizations and gestational age of diagnosis COVID-19. A total of 268 medical records were reviewed (67 cases and 201 controls). Cases were considered to be pregnant women with a diagnosis of HDP and controls were those pregnant women without a diagnosis of HDP hospitalized in the obstetrics and gynecology service of the “Hospital Regional del Cusco” in 2021, both groups met the inclusion criteria. Results: COVID-19 was associated with an increased risk of developing Hypertensive Disorders of Pregnancy ORa: 20.92; IC95% [7.94-55.11]; Preeclampsia constituted the main disorder associated with COVID-19 ORa: 15.41; IC95% [6.47 – 36.71]. Conclusions: COVID-19 was determined to represent a risk factor for the development of Hypertensive Disorders of Pregnancy and Preeclampsia in the “Hospital Regional del Cusco” in 2021.
