Background: OXA-48-like carbapenemases are rapidly expanding globally, and Peru is no exception. The gold standard test for their detection is the PCR and sequencing, but these are not accessible to health facilities in our area. Objective: To evaluate the performance of phenotypic methods for the detection of OXA-48-like carbapenemases in Enterobacterales, in terms of sensitivity and specificity. Methods and Materials: Six phenotypic methods were evaluated to detect the presence of carbapenemases in general and another 7 for the specific detection of OXA-48-like carbapenemases. Finally, the minimum inhibitory concentration (MIC) was evaluated. Results: Of the 159 isolates evaluated, 117 were OXA-48-like producers. 69.2% of these were Escherichia coli and 4 isolates were dual carbapenemase producers. Within the general carbapenemase detection tests, the CIM and the rCDM showed the best sensitivity and specificity of 99.2% and 100% (Kappa coefficient = 0.98). On the other hand, in the specific detection of OXA-48-like, the temocillin disk on MH agar showed the highest sensitivity with 94% (Kappa coefficient = 0.87). Regarding the MIC of meropenem, ertapenem and imipenem, a significant difference in medians (p < 0.001) was evident between OXA-48-like and strains that do not produce this resistance mechanism. Conclusion: Methodologies for detecting carbapenemases in general showed high sensitivity and specificity against OXA-48-like, while tests for specific detection of OXA-48-like obtained heterogeneous results. Regarding MIC, ertapenem stood out as a highly sensitive indicator for this resistance mechanism.
