Introduction: The pandemic caused by SARS-CoV-2 has affected around 250 million people. Peru is the country with the highest mortality rate. The progression, development and outcome of the disease are highly variable. One factor that could be relevant to the severity is the blood group. Studies in Latin America and Peru are very scarce and the distribution of blood groups is different. This research sought to describe the association between blood group and the development of severe COVID-19 in adults in a National Hospital in Lima between January and June 2021. Methodology: This was a cross-sectional, observational, analytical study in which medical records were reviewed. The population consisted of patients over 18 years of age admitted to the Rebagliati Hospital for COVID-19 between January and June 2021. The sampling type was simple random probabilistic. The exposure was the blood group, and the dependent variable was the severity of the condition, defined by the IETSI clinical practice guideline. An ordinal logistic regression was performed, from which the OR was calculated with a 95% confidence interval. Results: There was no statistically significant difference between the different blood groups and the severity of COVID-19 (p = 0.206). However, severity of disease was associated with male sex, 60 years or older and comorbidities with statistical significance (p = 0.032; 0.033; <0.001; respectively). Blood group O was the most prevalent, with 76.8%, followed by blood type A with 16.3%. Conclusions: In conclusion, no statistically significant association was found between blood group and the severity of COVID-19. This study can serve as a basis for future research seeking to study this phenomenon.
