Introduction: Adherence to pharmacological treatment of arterial hypertension is a global challenge, with compliance rates that vary widely and are influenced by various factors, such as comorbidities, harmful habits, and sociodemographic characteristics. Objective: To evaluate the factors associated with adherence to pharmacological treatment of hypertension in Peruvian patients, using data from the 2022 National Demographic and Health Survey. Materials and methods: Observational, retrospective, cross-sectional, secondary baseline study. Results: Older age (RPa =0.994, p < 0.001), having health insurance (RPa = 1.082, p < 0.001), purchasing blood pressure medication (RPa = 1.169, p < 0.001), having a diagnosis of diabetes (RPa =1.190, p < 0.001), and residing in urban areas (RPa = 1.073, p < 0.001) were significantly associated with greater adherence to HT treatment. In addition, smokingin the last 30 days (RPa = 0.899, p < 0.001) and consumption of alcoholic beverages (RPa = 0.938, p < 0.001) are associated with lower adherence. Conclusions: Sociodemographic characteristics, health access, comorbidities and harmful habits were found to be associated with therapeutic adherence in hypertensive patients.
