The present study aimed to determine the association between maternal knowledge about childhood vaccination and adherence to the immunization schedule in children under 5 years old at a health center in North Lima during 2024. It had a quantitative, non-experimental, correlational design, with a sample of 175 mothers who took their children to the San Pedro de Carabayllo Health Center. Surveys were used to assess maternal knowledge about vaccination, and checklists were used to measure adherence to the immunization schedule. Data were analyzed using IBM SPSS, applying the Chi-square test to determine the association between variables. Results showed that 54,3% of mothers had good knowledge about childhood vaccination, however, only 37,1% of them adhered to their children's vaccination schedule. The statistical analysis revealed a significant association between maternal knowledge and adherence to the schedule (p=0,003). Similarly, knowledge about vaccination by age had a significant association with adherence to the schedule (p=0,001), while knowledge about side effects and post-vaccination care did not show a significant association. Maternal knowledge about childhood vaccination is an important factor that influences adherence to the immunization schedule. However, other factors, such as access to health services and cultural beliefs, also play a crucial role. Therefore, it is recommended to strengthen education strategies, promote immunization programs, and communication aimed at mothers to improve vaccination coverage in the population.
