To determine the diagnostic accuracy of the BIDIAP index for appendicitis based on the operative report in patients between 5 and 17 years of age admitted to the José Cayetano Heredia Hospital, Piura. Material and methods: Observational, analytical, cross-sectional study with diagnostic test design between 2010-2023. A minimum of 383 patients to be included in the study was established by sample size calculation. Variables such as age, sex, clinical characteristics and blood tests were collected. By means of statistical analysis carried out in the Stata version 15 program, the categorical and numerical variables were summarized. The best cut-off point of the final value of the BIDIAP index was established to discriminate the presence of acute pediatric appendicitis by means of a ROC curve graph and calculation of the area under the curve. The values of sensitivity, specificity, positive and negative predictive value were reported. Results: The variables age, sex, time of illness, sensitivity in the right lower quadrant, anorexia or hyporexia, nausea or vomiting, pain migration, and systemic inflammation index were variables significantly associated with pediatric acute appendicitis. The area under the curve of the BIDIAP Index for the diagnosis of pediatric acute appendicitis was 0.95 (95% CI 0.90-0.98). Cut-off point ≥ 4 identifies 99.2% of patients with pediatric acute appendicitis, with a sensitivity, specificity, positive predictive value, and negative predictive value of 90.4, 91.0, 99.2 y 43.5, respectively Conclusions: The BIDIAP index has adequate sensitivity and specificity for the detection of acute appendicitis in patients between 5 and 17 years of age.
