With the objective of comparing the effectiveness of third stage of labor directed vs. manual third stage of labor in complications of the immediate postpartum period in patients treated at the Santa Isabel District Hospital, El Porvenir 2018, a retrospective, analytical and cohort study was carried out, with 2 groups, one of exposed and the other was not exposed, with a sample of 53 medical records. for each group. Results: It was found that the characteristics of the puerperal women were: in vaginal childbirth and surgical childbirth from 20 to 25 years old 74% and 79% according to age respectively, multiparous (51%) in vaginal childbirth and primiparous (45%) in surgical childbirth, hemoglobin normal prepartum in both types of births (92% and 98% respectively, complications such as postpartum bleeding were greater than 500cc in directed third stage of labor and 200 to 500cc in manual third stage of labor, hypotonia occurred in 58% in third stage of labor directed and without hypotony 96% in manual third stage of labor, in patients treated at the Santa Isabel District Hospital 2018 Concluding that there is greater effectiveness in directed third stage of labor vs. manual third stage of labor, with a high statistical significance p < 0.001
