Objective To determine whether surgery as initial treatment in clinical stage IV breast cancer improves overall survival compared to other initial treatment. Material and methods: A systematic review and meta-analysis of the type of intervention was carried out following the Cochrane methodology. The study population was studies that cover patients diagnosed with Breast Cancer with clinical stage IV that compare primary surgery vs. non-primary surgery. A total of 55 articles were identified in the following databases: Pubmed, Scopus, EBSCO, Web of Science; In addition, which were stored in the Rayyan tool, these were identified and examined, leaving 03 clinical trial type articles. Results: The total synthesis of the data showed an RR of 1.02 with a 95% CI of 0.91 – 1.14; When evaluating heterogeneity, Chi2 was found: 1.68 with P:0.43, which indicates that there is no heterogeneity and with I2 of 0% indicating almost zero heterogeneity. Conclusions: Surgery as initial treatment in clinical stage IV breast cancer does not improve overall survival.
