To demonstrate the association between the number of perioperative transfusions (0, 1-3 and more than 3) and the type of gastrectomy (total vs. subtotal) with intrahospital pneumonia in gastrectomy with extended D2 lymphadenectomy in advanced gastric cancer. Material and methods: An observational study, type cases (with hospital-acquired pneumonia= 33) and controls (without hospital-acquired pneumonia=159) nested in a cohort of 192 patients who underwent gastrectomy with extended D2 lymphadenectomy for advanced gastric cancer. The X2 test or Fisher's exact test was used for categorical variables, and the logistic regression test was used for multivariate analysis. Results: By the bivariate analysis, there was an association between the number of transfusions and the presentation of postoperative pneumonia (p=0.04) and the type of gastrectomy was not a statistically significant factor (p=NS). In the multivariate analysis, there was a statistically significant association between the number of transfusions and the presence of intrahospital pneumonia (p=0.02, OR=2.76, 95% CI= 1.175 - 6.483), but the type of gastrectomy was not an associated factor (p= 0.326, OR=1.56, 95% CI= 0.64 – 3.83). Conclusion: The number of blood transfusions influences the presentation of hospital-acquired pneumonia in surgery for advanced gastric cancer.
