Currently, the evidence of intrauterine transmission from mother to fetus and its association with respiratory distress in the newborn is scarce. Objectives: To determine if there is an association between SARS-CoV-2 infection in the second half of pregnancy and the respiratory distress in the newborn. Methods: Systematic review of primary studies in the MEDLINE/PUBMED, EBSCO, SCOPUS, SciELO, Science Direct, LILACS, and Google Scholar databases. To synthesize the data, the Forest plot of the meta-analysis was used, taking the risk ratio as an index of the effect size, weighted using the Mantel Haenszel method and assuming a fixed effects model using the RevMan program (Review Manager 5.3). Results: 251 articles were obtained that were analyzed according to the PRISMA 2020 diagram. 18 studies were reviewed independently and 5 relevant studies were selected for analysis. The prevalence of respiratory distress in the newborn of mothers infected with SARS-CoV-2 in the second half of pregnancy was 2.6% (72) in a population of 2,759 pregnant women. The included studies did not show considerable heterogeneity (I2=26%). A relative risk of SARS-CoV-2 infection during pregnancy for newborn respiratory distress of 1.97 (95% CI: 1.41 – 2.76) was determined, although it was not significantly associated (p = 0.25). Conclusion: There is no significant association between SARS-CoV-2 infection in the second half of pregnancy and respiratory distress in the newborn.
