Determine the diagnostic value of the physical examination compared to the cardiac biomarker (NT-proBNP) in patients of the medicine service of the Jorge Reategui Delgado Hospital in the period November 2023 to May 2024. METHODOLOGY: An observational study of diagnostic tests was developed in all patients with suspected acute heart failure evaluated at the Jorge Reategui Delgado-Piura Hospital II of Essalud. The patients underwent an initial evaluation in which epidemiological data, clinical evaluation were obtained, and the presence of a positive myocardial biomarker (NT-pro-BNP) was evaluated. At the end of the collection, these data were entered into an Excel database to continue coding and statistical analysis and interpretation were carried out to meet the objectives of this thesis. RESULTS: Of the 210 patients who arrived at the medical service during the study period, they were diagnosed with acute heart failure using the Framingham 159 and Boston 164 criteria.The NT-proBNP test was applied to the same group of patients in the same period, considering a biomarker value greater than 4000 pg/ml as positive, with which the diagnosis of acute heart failure was established in 190 patients. The number of patients diagnosed with the Framingham and Boston criteria was slightly lower than those diagnosed with NT-proBNP. There were 20 patients who were not diagnosed with heart failure because they did not meet the Framingham and/or Boston criteria nor were positive cardiac biomarker values obtained. CONCLUSIONS: The results of this study show a notable similarity in the diagnostic efficacy between clinical assessment, based on the Framingham or Boston criteria, and the use of the NT-proBNP biomarker for the diagnosis of acute heart failure (AHF)
