Objective: To compare the effect of the rotary and reciprocating system on post endodontic pain in patients treated at the Miguel Grau Polyclinic, Piura 2023. Methods: An experimental and longitudinal study was carried out in 50 male patients with a diagnosis of pulp necrosis in lower molars and indication for endodontic treatment. The patients were randomly assigned to the rotary system (25) and reciprocating system (25). Postoperative pain was assessed using the VAS pain scale at 6, 12, 24, 48 and 72 hours. Both groups received endodontic treatments in a single visit. Student's t-test was used to compare the averages of postoperative pain at the different times, with significance at 5% (p < 0.05). Results: When comparing the means of both systems evaluated, significant differences (p < 0.05) were found between the postoperative pain scores at 6, 12, 24 and 48 hours. However, no significant statistical difference was found between both systems at 72 hours (p >0.05). Conclusions: Postoperative pain with the reciprocating system is less at 6, 12, 24 and 48 hours. However, there is no difference between the two systems at 72 hours
