Helicobacter pylori is a microorganism that causes gastritis and peptic ulcer. Socioeconomic level and living conditions are social determinants that represent the risk of having this infection. Objective: The main objective of the study was to evaluate the greater validity of the modified Kyoto scale compared to the Kyoto scale for the diagnosis of Helicobacter pylori. Methods: An analytical, cross-sectional, diagnostic test study was carried out in 220 patients in a third level public hospital in Peru. An Olympus GIF HQ-190 endoscope was used to assess the gastric mucosal surface pattern. In each patient in the study, biopsies of the gastric antrum and body were obtained for microscopic study. Results: 220 patients were evaluated, 128 of them had H. pylori. The positive and negative likelihood ratio values of the modified Kyoto scale were 1.89 and 0.57 while the positive and negative likelihood ratio values of the Kyoto scale were 1.53 and 0.72. Conclusions: The modified Kyoto scale did not have greater validity than the Kyoto scale for the diagnosis of Helicobacter pylori.
